RESUMO

BACKGROUND: Hemangioma is a vascular benign tumour of endothelial origin. It appears commonly in the first decade of life with increases incidence in females. Hemangioma is not common to happen in the oral cavity and it is extremely rare to appear in the labial vestibule. CASE SUMMARY: We present a case of an 11-year-old girl who complained of a painful, slowly growing mass which was consistent with the capillary hemangioma in the left mandibular vestibule. Vascular tumor such as hemangioma in the mandibular vestibule is extremely rare; hence, the clinical definitive diagnosis is very challenging. Therefore, radiographic imaging and histopathologic analysis are crucial to reach to the final diagnosis for proper management. CONCLUSION: Comprehensive clinical evaluation, proper diagnostic imaging and microscopic analysis of the mass establish a precise diagnosis of the hemangioma for better management.

BACKGROUND: Digital radiography has recently been used in dentistry as a substitute for conventional film radiography worldwide. Digital imaging has many advantages and provides new possibilities for recording and interpreting radiographic data. This system uses different types of digital receptors. AIM: To detect the frequency, type, and reasons behind the appearance of intraoral image artifacts acquired by photostimulable phosphor plates (PSP). METHODS: This retrospective descriptive study was conducted in the oral and maxillofacial radiology unit of the dental clinics of the College of Dentistry, Princess Nourah University (PNU). All intraoral digital radiographs were acquired using (Gendex Expert DC., United States) an intraoral X-ray machine with 7 -mA, 65-kVP using a PSP system (Soredex DIGORA Optime imaging plate) and laser scanners (Soredex DIGORA Optime), which can house all sizes of reusable intraoral PSP sensor plates with image acquisition software (MIPACS Dental Enterprise viewer 3.2.2). A total of 50000 intraoral radiographs were retrieved from the clinical database from April 2018 to April 2020 to evaluate the reason, type, and solutions to these image artifacts. RESULTS: Overall, 50000 intraoral digital radiographs were acquired in a two-year-period; that is, from April 2018 to April 2020. Of these, 3550 (7.1%) retakes were performed due to the presence of image artifacts. Periapical radiographs were the most common image type of intraoral retakes (80.8%). Imaging artifacts were divided into three categories: operator, plate and scanning errors. Out of 3550 retakes, 5%, 1.37%, and 0.73% were related to operator, plate, and scanning errors, respectively. The cone cut was the most common operator error (988 images), Bite marks were the most common plate error (276 images), and delayed scanning artifacts were the most common scanning errors (145 images). The calculated kappa value for interobserver reliability was 0.99, indicating almost perfect interobserver agreement. CONCLUSION: Our study discussed intraoral image artifacts that were characteristic of PSP, where the most common artifacts were bitemarks, image size reduction, scratches, and delayed scanning.

INTRODUCTION: Palatine tonsils are part of the mucosa-associated lymphoid tissue, located in the oropharyngeal region. Although these tissues protect the body from foreign intruders, they are more prone to infections due to their anatomical structure and location. For instance, the differential diagnosis of a white lesion on the palatine tonsil can range from benign to malignant lesions. Oral lymphoepithelial cysts commonly arise as painless, yellowish nodules on the floor of the mouth and the ventral or lateral surface of the tongue. Case Presentation. This paper presents a rare case of an unusual site of a lymphoepithelial cyst (LEC) in the oral cavity. The lesion was located in the tonsil of a 20-year-old woman with a chief complaint of a painless, white lump in the back of the mouth for nine months. Discussion. The differential diagnosis of a white lesion on the palatine tonsil is caused by several factors, such as bacterial, viral, and fungal infections; trauma; stones; cysts; abscess; or cancer. In this case, both the clinical presentation and extra- and intraoral examinations were highly associated with LEC. Oral LEC etiopathogenesis is uncertain, and several theories have been proposed to discuss the causes of LEC. In addition, oral LEC could be monitored without surgical intervention if the nodule is asymptomatic. CONCLUSION: We emphasize the importance of a thorough clinical examination of oral and oropharyngeal lesions, which are usually neglected.

BACKGROUND: Herpes simplex virus (HSV) is a highly infectious pathogen that is easily transmitted via the bodily fluids of an infected individual. This virus usually affects individuals older than six months of age, and rarely causes lesions or symptoms in younger patients. CASE SUMMARY: We present the case of a five-month-old healthy girl who presented with painful herpetic gingivostomatitis and perioral vesicles. We discuss the pathophysiology of primary HSV infection and the effect of maternal antibodies on the infant's immune system. In addition, we explain the diagnosis, management, and prognosis of HSV infection in young infants. CONCLUSION: This case highlights the importance of early diagnosis and management of HSV infections to decrease the risk of developing severe complications and death.

INTRODUCTION: intramuscular hemangioma (IMH) is a relatively rare benign vascular tumor of the skeletal muscles. It shows less than 1% of all soft tissue hemangiomas. In the head and neck area, it occurs usually in the masseter muscle followed by temporalis and sternocleidomastoid muscles. PRESENTATION OF CASE: we present a case of 25-year-old male patient with a chief complaint of slowly growing facial swelling in the left zygomatic area. Clinical, imaging and histopathological evaluation lead to the diagnosis of intramuscular hemangioma in the zygomaticus major muscle. DISCUSSION: IMH in the zygomaticus muscle is very rare; hence, the clinical diagnosis of IMH is challenging. Different diagnostic procedures can be used such as CT and MRI. In addition, the ideal therapy for esthetic disfiguring IMH in the head are is the complete surgical excision of the lesion. Through the review of literature and to our knowledge this case is the first report of intramuscular hemangioma in the zygomaticus muscle. CONCLUSION: IMHs are rare in the head and neck area and must be considered in differential diagnosis of isolated muscle mass in this region.

RESUMO

CXCR3 is a well-known receptor involved in immune cell recruitment and inflammation. Pathological inflammation leads to pain stimulation and hence nociception. Therefore, we decided to review the recent research on CXCR3 to identify its precise role in the modulation of pain in a variety of clinical conditions targeting various regions of the body. Studies were selected from PubMed Medline, which relate CXCR3 to the progression of diseases with either bone cancer pain, neuropathic pain, cystitis pain, osteoarthritis and rheumatoid arthritis pain, dental pain, in particular, periodontitis and pulpitis. In all the diseases studied, a high prevalence of CXCR3 and/or its ligand were identified where CXCR3 is a key player in the pathophysiological process of many inflammatory conditions. CXCR3 and its ligands, particularly CXCL10, modulate nociception via actions in the dorsal root ganglia and dorsal horn of the spinal cord, in cases of bone cancer pain, neuropathic, and joint pain. However, with the other studied disease, no direct link to pain has been made, although it contributes to the pathological progression of the diseases and hence would be a causal factor for the pain. Furthermore, CXCR3 appears to play a role in desensitizing the opioid receptor in the descending modulatory pathway within the brain stem as well as modulating opioid-induced hyperalgesia in the dorsal horn of the spinal cord. Further research is required for understanding the exact mechanisms of CXCR3 in pain modulation centrally and peripherally. A greater understanding of the immunological activities and pharmacological consequence of CXCR3 and its ligands could help in the discovery of newer drugs for modulating pain arising from pathogenic or inflammatory sources. Given the significance of the CXCR3 for nociception, its utilization may prove to be beneficial as a target for analgesia.
